Getting Started is Straightforward.

Get A Referral

Contact your primary physician and healthcare providers to find out if you or your loved one are ready for palliative care. Your doctor will listen to your situation and explain how we may be able to help. There is no commitment required at this stage.

We Assess and
Plan Together

Once your doctor has referred you or your loved one to us, our team meets with you and your family to understand your symptoms, your priorities, and your wishes. We develop a care plan that is built around your priorities.

Ongoing Care
at Home

We provide regular home visits, telephone support, and coordination with your other medical specialists.

What is Palliative Care?

Palliative care is specialised medical care for people living with serious illness. It focuses on providing relief from symptoms, pain, and the stress that serious illness creates — for both the patient and the family.

It is not about giving up on treatment. Palliative care can be provided at any age, at any stage of illness, and alongside curative treatments. Many patients receive palliative care for months or years before the end of life.

The word “palliate” comes from the Latin pallium — to cloak, to cover. We provide a layer of support, comfort, and care that the standard medical system is not designed to offer.
